At CornerstoneVision, our goal is to never turn anyone away who is in need of our counseling and psychological services.

INSURANCE

We are a provider for many insurance companies. Please call our office to get the most updated information and to learn which of our therapists is a provider for your insurance.

COMMUNITY SUPPORT

For those clients who do not have mental health insurance benefits, CornerstoneVision offers an adjusted fee scale by application. We’ve also partnered with local churches to offer payment assistance to self-pay clients. Contact our office for more details.

We accept MasterCard, Visa, Discover, HSA/Flex Cards, and personal checks.

GOOD FAITH ESTIMATE

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.

 Under the law, healthcare providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.


You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.

Make sure your healthcare provider gives you a Good Faith Estimate in writing at least one business day before your medical service or item. You can also ask your healthcare provider and any other provider you choose for a Good Faith Estimate before you schedule an item or service.


If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.

Make sure to save a copy or picture of your Good Faith Estimate.

CANCELLATION & CHECK POLICIES

Cancellation

CornerstoneVision Counseling requires 24-hour notice to cancel an appointment.

There is a $45.00 Late Cancel Fee for less than 24 hours ' notice. For Dr. Sowles’ clients, this charge increases to $145.00 if the late canceled appointment slot cannot be filled.

Call extension 110 to cancel your appointments. You can leave a voicemail message 24 hours a day.

Intake/ First Session Fees

$150.00 for a licensed therapist

$115.00 for a pre-licensed therapist

Paying by Check

For self-pay clients, checks should be made payable to CornerstoneVision Counseling.

Please note that any checks returned for insufficient funds will incur a $35.00 charge payable to CornerstoneVision Counseling, in addition to any fees your bank may charge.
